Do all athletes need to fill out medical clearance paperwork?
Yes. All returning, transfer and new incoming student trying out for a varsity and/or a club sport at Endicott College is required to complete medical clearance paperwork each year through Endicott Athletic Training. Select the hamburger menu on the top right corner of the webpage. Instructions for completing the paperwork are located under the Student-Athlete Services- Athletic Training - Medical Clearance Forms. Please find your respective column and open the first link for instructions.
Can I play without completing the paperwork?
No. Athletes will not be cleared to participate at any level in any Endicott team sponsored practice, game or workout without completing medical clearance paperwork in full. The medical clearance paperwork needs to be received, reviewed and approved by the athletic training staff.

What is included in the medical clearance paperwork?
The medical clearance paperwork varies slightly for New Incoming/Transfer Student-Athletes versus Returning Student-athletes. The paperwork includes up-to-date pre-participation physical exam, a copy of a current insurance card, sickle cell paperwork (if applicable) and medical exception paperwork (if applicable) and completion of your athlete profile on SportsWareOnline.

What is SportsWare Online?
SportsWare is a secure, online injury tracking database. This is the portal where Endicott Athletic Training will collect necessary medical information about each student-athlete. If you are an Incoming/Transfer student, you will need to create and complete a profile as part of the medical clearance process. If you are a returning student-athlete, you should already have a SportsWare account log-in and password. Did my forms arrive? Why hasn’t someone called me back?
The Endicott Athletic Training office is closed from June 1st through July 31st. If you should need assistance feel free to contact Abbey Cahill at acahill@endicott.edu or James T. Daley at jtdaley@endicott.edu. All athletic trainers will be periodically checking their e-mail and voicemail throughout the summer.
What are the deadlines for medical clearance paperwork?
The deadline for submitting all medical clearance paperwork to Endicott Athletic Training is August 1, 2017.
What happens if I do not submit my forms by the deadline?
If you do not submit your forms by the deadline for your sport there will be a 48 hour processing period. This means that you will be unable to practice or participate in a team sponsored workout for 48 hours until the athletic training staff is able to look over and approve your paperwork.
What if I do not submit my forms by the time my team checks-in for the Fall?
If your paperwork has not been submitted by the time your team checks-in OR parts of your paperwork are still missing by the time you check-in, you will not be permitted to check-in. This means you will not be permitted to receive your room keys, move-in to your dorm or participate in any team sponsored practices and/or workouts. You will be asked to return home until the rest of the student body is scheduled check in for the semester.
As a new incoming student, why do I need a physical within 6 months of starting my participation in athletics?
The NCAA ByLaw 17.1.5 states that “prior to participation in any practice, competition or out-of-season conditioning activities, student-athletes who are beginning their initial season of eligibility and students who are trying out for a team shall be required to undergo a medical examination or evaluation administered or supervised by a physician (e.g., family physician, team physician). The examination or evaluation must be administered within six months prior to participation in any practice, competition or out-of-season conditioning activities.”
As a returning student-athlete, why do I need an up-to-date physical every 12 months?
Many changes to one’s health can occur throughout a collegiate career. Endicott College athletic training staff requires an updated physical every 12 months (from date of exam) for returning students to ensure each athlete is in good health and can participate in collegiate athletics without restrictions.
I have had an injury in the last 12 months, what additional documentation do I need.
In addition to an annual physical form from your primary care physician (PCP), we also require a separate note from any specialist/orthopedist you have seen, clearing you for participation in athletics.
Why do I need to provide my personal insurance information?
In cases of emergency, where you become ill or seriously injured, the athletic training staff will send copies of your personal insurance information along with you to the doctor or emergency room. Any bills incurred will be billed through your primary insurance company first.
What are medical exceptions?
The NCAA bans student-athletes from taking certain classes of drugs because they are harmful to the body and can create an unfair advantage in competition. Some legitimate medications, particularly those for ADHD and asthma, contain NCAA banned substances and may be necessary for the student-athlete to take in order to support their academics and/or their general health. These medications are considered medical exceptions.
Why do I need to provide additional information if I am taking a medication on the banned substance list?
If a student-athlete taking a medication on the medical exception list were to be randomly selected for drug testing, he/she would produce a positive test. In order to avoid ramifications of taking a banned substance and producing a positive drug test, the student athlete would need to provide the athletic training staff with proper additional documentation. This paperwork must include 1) an initial note from the prescribing physician clarifying the student’s diagnosis and the need for the banned medication, 2) supplemental diagnostic tests, 3) an annual follow up physician’s note and 4) a copy of the student-athletes most recent prescription.
Are these medical forms separate from forms I need to hand in to the health center?
Yes. These forms are separate from the forms the health center requires. The health center requires a physical and an immunization record. You can make a copy of your physical and send one copy to the health center and one copy to the athletic training staff however; you must also fill out the remaining paperwork required by the athletic training staff.
What is sickle cell trait?
Sickle cell trait is not a disease. Sickle cell trait is the inheritance of one gene for sickle hemoglobin and one for normal hemoglobin. Sickle cell trait will not turn into a disease. It is a life-long condition that will not change over time.
During exercise, red blood cells containing oxygen circulate through an athlete’s blood to supply the brain, organs and muscles with adequate oxygen to continue exercising. In those athlete’s with sickle cell trait, the red blood cells change shape from round to a quarter-moon, or “sickle” shape. These sickle red cells may accumulate in the bloodstream during intense exercise, blocking the normal blood flow to the tissues and muscles. Athletes with this trait have displayed physical distress, weakness, fatigue, collapse and even death during exercise.
Can I fax my forms in?
