NCAA Medical Exemptions Policy and FAQ’s
Freshman/Transfer Student Athletes: you will receive an Endicott College Health Form from the Student Development Office. Please complete the form and make two copies. Send the original to the Health Center, one copy to Athletic Training, and retain a copy for your records.
Please complete all of the necessary paperwork and return it to Endicott Athletic Training no later than August 1st, 2010. List your sport (s) on the front of the envelope.
